Nasopharyngeal carcinoma (NPC) presents persistent therapeutic challenges due to high rates of local recurrence and distant metastasis. Identifying central oncogenic drivers within NPC altered signaling networks is critical for developing targeted pharmacological strategies. Integrated bioinformatics analysis combined with machine learning classification frameworks established denticleless E3 ubiquitin protein ligase homolog (DTL) as a primary hub gene and prominent oncogenic driver in NPC tissues. DTL operates as the substrate-recognizing subunit of the Cullin-RING E3 ubiquitin ligase 4 (CRL4) complex. Overexpression of DTL accelerates the ubiquitin-mediated degradation of key regulatory proteins, thereby dismantling essential cell cycle checkpoints and apoptotic signaling pathways in malignant cells.

Functionality of the CRL4-DTL complex depends strictly on cullin neddylation, an essential post-translational modification governed upstream by NEDD8-activating enzyme (NAE). In this biochemical cascade, NAE catalyzes the ATP-dependent activation of NEDD8, transferring it to downstream E2 conjugating enzymes prior to final ligation onto the Cullin 4 scaffold. Cullin neddylation induces a conformational transition that positions the bound ubiquitin-E2 complex in proximity to target substrates. Without neddylation, the CRL4-DTL ligase remains functionally dormant. Pharmacological disruption of NAE therefore represents an attractive intervention point to systematically shut down CRL4-DTL-dependent protein degradation.

Targeting this upstream enzymatic node, the small-molecule inhibitor MLN4924 (Pevonedistat) engages the active site of NAE. Structural computational modeling and molecular docking assays demonstrate that MLN4924 binds stably within the NAE nucleotide-binding pocket with a binding energy of -7.3 kcal/mol. This high-affinity interaction blocks the formation of the NAE-NEDD8 thioester intermediate, halting downstream transfer of NEDD8 to Cullin 4. Deprived of neddylation, the CRL4-DTL complex loses its structural activation state. Consequently, the ligase fails to recruit and polyubiquitinate its primary tumor suppressor substrates, p21 (CDKN1A) and p53 (TP53).

In treated NPC cell lines, inhibition of CRL4-DTL by MLN4924 directly prevents the proteasomal clearance of p21 and p53. Under baseline conditions, high DTL turnover maintains minimal baseline steady-state levels of p21 and p53. NAE blockade reverses this phenotype, leading to rapid intracellular stabilization and continuous accumulation of both tumor suppressors. Accumulated p21 binds and inhibits cyclin-dependent kinase complexes, arresting cells at the G2/M phase boundary. Concurrently, elevated p53 triggers canonical intrinsic apoptotic signaling cascades, evidenced by increased cleavage of poly(ADP-ribose) polymerase (PARP) and caspase-3. In vitro functional assays confirm a dose-dependent reduction in NPC cell viability, clonogenic survival, and migratory capacity.

Translational evaluation using C17 patient-derived xenograft (PDX) murine models validated these biochemical findings in vivo. Administration of Pevonedistat led to statistically significant reductions in tumor volume and weight compared to vehicle control cohorts. Immunohistochemical analysis of harvested xenograft tissues confirmed elevated intratumoral concentrations of p21 and p53 alongside marked reductions in Ki-67 proliferative indices. Crucially, systematic safety monitoring demonstrated minimal organ histological alterations and no significant hematological toxicity or weight loss in treated animals. These observations indicate that NAE targeting via MLN4924 suppresses NPC growth by stabilizing p21 and p53 through CRL4-DTL inactivation.

Pevonedistat (MLN4924) selectively inhibits NEDD8-activating enzyme (NAE), disrupting cullin neddylation required for E3 ubiquitin ligase activity. In nasopharyngeal carcinoma, NAE inhibition functionally inactivates the CRL4-DTL complex. Blockade of CRL4-DTL prevents the degradation of tumor suppressor proteins p21 and p53, triggering accumulation of both substrates. Elevated p21 and p53 induce G2/M phase cell cycle arrest and initiate apoptosis, markedly impairing NPC cell proliferation and colony formation.

Structural Binding and Functional Disruption of the CRL4-DTL Axis

Computational docking simulations established that MLN4924 binds the NAE active site with a predicted binding affinity of -7.3 kcal/mol. This tight interaction blocks the enzymatic transfer of NEDD8 to Cullin 4. Neddylation of Cullin 4 ceased in treated NPC cells within hours. Inactivation of the catalytic scaffold functionally disassembled the CRL4-DTL ligase complex. Without enzymatic activity, ubiquitin transfer to substrate proteins was prevented. The clearance of p21 and p53 was blocked at nanomolar concentrations.

Induction of G2/M Phase Arrest and Apoptosis in NPC Models

Stabilization of p21 directly altered cell cycle kinetics in cultured NPC lines. Flow cytometry showed a marked accumulation of cells in the G2/M phase. Cellular viability dropped sharply after exposure. Concurrently, stabilized p53 activated caspase-dependent apoptotic pathways—a response confirmed by PARP cleavage assays. MLN4924 treatment suppressed long-term colony formation across multiple tested cellular models.

In Vivo Tumor Suppression in Patient-Derived Xenograft Models

In vivo evaluation utilized C17 patient-derived xenograft models to assess systemic anti-tumor activity. Dosing schedules resulted in significant tumor volume reduction compared to vehicle controls. Intratumoral analysis showed increased p21 and p53 expression in harvested tissue samples. Toxicity profiles remained favorable. Body weight trajectories and hematological parameters showed no severe drug-related adverse events during treatment.
